These forms are ONLY for people who already have appointments booked with us,
but who have not received any questionnaires - or - who have lost them:
  1. Download the appropriate questionnaire or form with Adobe Viewer.
  2. Print it (you can't complete it on line).
  3. For questionnaires, complete them and fax them to 807.622.0273 or bring them to the office.
  4. For appointment forms, call the office at 807.622.0333 to check the appointment details.
